Is your community trying to forge housing-based partnerships between homeless service providers and local hospitals? You're not alone! Please join this online strategy session to share your experiences, hear new ideas and brainstorm with leaders from other communities grappling with the same issue!

This call will feature examples and discussion of the following practices:

  • Building relationships with local hospitals
  • Agreeing upon a release for patient information
  • Methods of transferring Vulnerability Index information from hospitals to your registry
  • Best practices for collecting this information in hospitals and connecting patients to appropriate housing and services
